The Bad Date: Vincent Vega takes Mrs Mia Wallace out to a 1950s-themed restaurant called Jack Rabbit Slim’s, where they proceed to dance the night away. Then they head back to Vega’s, where Mia mistakenly snorts a load of heroin.
Worst Moment: Overdosing, Mia goes into shock and Vega is forced to administer an adrenaline shot straight to the heart.
How To Make It Romantic: Administering the kiss of life would have been a far more romantic way of resuscitating Mia.

I think Evil Dead 2 ranks as probably the worst, if you have to decapitate your demonically-possessed girlfriend, then its fair to say the relationships going nowhere.
