It's not a very well known film but pretty much any scene in Agnieszka Holland's Nazi-era suspense drama Europa Europa (1990) would have fit the bill in terms of nail bitingly tense undercover scenes. Just the plot alone - a Jewish boy disguises himself as a German and joins the Hitler Youth to avoid persecution from the Nazis - would give you an idea of how tense the film gets. One scene that comes to mind particularly involves the now-undercover protagonist being called out to stand in front of a Nazi class learning about how to identify a Jew.

The apple/vomit scene from "Sound of my voice" would have been on my list.
